Requesting a Transcript
Q: How do I request an official transcript?
A: You may obtain an official copy of your Pike-Lincoln
Technical Center transcript by submitting a request in writing
with your signature. A $3 fee is required at the time of request.
The fee may be paid by check or cash. Requests cannot be taken
by phone.

• The date of your request.
• Your full name, as well as any names previously used
• Your social security number, also known as your student
number
• Your complete address
• Your date of birth
• The address where the transcript is to be sent
• Your daytime phone number
• Your legal signature authorizing the release of your records

Q: Can you request a transcript over the phone/ e-mail?
A: Requests cannot be taken by phone or e-mail in order
to ensure confidentiality.
Q: Can requests be faxed?
A: You can fax your request to 573-485-2388. Transcripts
not picked up in person can be faxed or mailed. Transcripts are
$3 per copy. They can be faxed for $10 per copy. Payment must
accompany each request received. Payment can be made via check
or cash. Please contact the Director's office at 573-485-2900
between 8-4 with any additional questions.
Q: Can my mother stop by and pick up a copy of my transcript?
A: A parent or spouse can obtain a copy of a student's
transcript only if the student provides them with written, dated,
and signed permission for release of the transcript. The person
picking up the transcript must also provide photo ID. Federal
law prohibits release of grades or academic record information
to anyone but the student without this special permission.
